Medication Aide Coordinator Plays Key Role in Assisted Living Houston Home Health Agency

Residents move to a senior independent living community with the desire to stay independent as long as possible. Treemont introduced the concept of “Independence Plus on our campus. Seniors who need help with activities of daily living can receive personal assistance services (PAS)and Medicare home health services in their apartment instead of moving to a licensed assisted living facility. One key Senior Allegiance Program is Medication Management. We are fortunate to have such  a caring “jewel” as Medication Program Coordinator Diamond Texada. Diamond started a year ago as a weekend med aide before assuming the leadership role when Cindy Harrell became the PAS Coordinator. In her role as Med Aide Coordinator, Diamond provides routine daily medications, prescription and non-prescription, records vital signs such as temperature, blood pressure and blood sugar per medical staff and ADON Ana Vazquez.

There are several levels of medication management. The most basic program is for a resident to be reminded twice daily to take their pills The more robust program involves the “management” of the program: prescription refills; taking vital signs; coordinating with doctor if there is an issue with the resident’s vital signs; delivery of the pharmacy prescription to the resident. The benefits of this more robust program is that that Senior Allegiance manages the communication between the physician and families as well as working with the pharmacist monthly when prescriptions are reordered to make sure there are no contraindications between the resident’s prescriptions.

Texada was raised in Houston by her parents with 3 older sisters and one younger brother. She has two adorable nephews with whom she enjoys spending her free time. Diamond graduated from Jack Yates High School in 2011 and worked as a daycare assistant teacher. Diamond enjoys shopping, reading, listening to different genres of music, dancing, working out and jogging. She enjoys creating new memories with friends and family, laughing and remember great times.

Texada says: One of my passions has always been to help others; I was tired of having job that gave me nothing in return. I wanted a job that I could be proud of, and feel good about doing every day. Even when it gets stressful I remember that I am helping. I find great commonalties with my residents each day I greet them with a “Good Morning Sunshine!” where Mrs. Hanson always replies “It’s too early, I’m not shining yet.” Working with people is so rewarding and I really want to be able to feel the joy you feel when working with vulnerable populations. To be their advocates, friends, also I hope there is a lot I could learn from them and carry that with me through my life. They always have great stories of their lives along with great wisdom to share. Just seeing them smile and look forward to seeing you is very pleasing! Yulisa Barrera is the Medicare Home Health Care Scheduler & Billing Manager for Senior Allegiance. She is in charge of scheduling the visits for nurses and Home Health Aides, including coordinating visits with admitting Nurses Karina Walker and Ana Vasquez.  Another key Yulisa role is sending the claims to Medicare for each patient receiving Home Health services. She also does the Intake for patients to make sure they have all the requirements to be admitted under Home Health. That means reviewing all the documentation such as Physicians Orders, Eligibility Progress Notes and Face to Face sheets.

This permits our residents to maintain an independent lifestyle, and keep a larger living unit, often at a lower cost than comparable assisted living facilities. Prior to Treemont, Yulisa worked at Methodist Hospital Medical Center for two years as a Patient Care Associate II. She then worked from 2008-2009 Treemont Home Care as a Nurse Assistant before joining the Senior Allegiance Home Health Care Team in 2011. Yulisa has a CNA license and a Phlebotomy Certificate. Yulisa is from El Salvador, where her parents and two brothers still reside. She has a lovely husband and two beautiful daughters.

 

Yulisa says “I love to work with seniors because I learn a lot of things from them. They have wisdom, nice histories and they are just looking for people who can give them a few minutes of companion to talk about their good times. I remember with love those patients that had an impact in my life. I truly feel blessed to work around seniors. One of my patients would sing  to me: ‘You are my sunshine my only sunshine you make me happy when skies are gray’ every time I walked into her apartment. She made my day every time I went to see her! Yulisa also remembers a gentleman for whom she assisted his sweet wife. A year later she passed and then he became her patient. He was so sweet and kind. She remembers him saying to her, “What will I do without you?”
